How to Complain to NTC About a SIM Registration Problem
Choose the correct government route, build a useful evidence file and submit an unresolved carrier registration complaint without exposing your OTP, ID or account data publicly.
Report the registration issue to your carrier first and save its case reference. If the telecommunications-service problem remains unresolved, use NTC’s official Telco Complaint form. Use the National Privacy Commission instead for an alleged personal-data violation, and NTC’s separate spam/scam form for a malicious text or call.

NTC regulates telecommunications services. NPC adjudicates complaints involving the Data Privacy Act and related issuances. One incident can involve both, but each complaint should identify the issue within that agency’s role.
When an NTC telco complaint may be appropriate
- The carrier repeatedly refuses or fails to register a SIM even though you supplied the requirements it currently publishes.
- An official portal or assisted channel cannot complete registration and carrier support has not resolved the issue.
- The carrier gives conflicting registration-status or ownership information.
- A properly reported SIM loss, deactivation request or registration-detail problem remains unresolved.
- You cannot obtain meaningful carrier assistance for a Home WiFi, pocket WiFi, eSIM or accessibility-related registration problem.
- A carrier or authorized channel attempts to charge for the registration process required by RA 11934.
An NTC complaint is not a guarantee that a SIM will be activated, a deactivated number will be recovered or a disputed ownership claim will be decided in your favor. NTC and the carrier will evaluate the facts.
Contact the carrier and create a written trail
Starting with the carrier is practical because it can inspect the account and fix many registration errors directly. It also gives NTC a clear record of what was attempted.

Do not use unofficial social-media accounts that ask for an OTP. Navigate to chat or support from the carrier’s own site or app.
What to prepare before filing
- Your full name/account name and a safe alternate contact method.
- The serving network and affected mobile or account number.
- The SIM product type: prepaid, postpaid, eSIM, Home WiFi, pocket WiFi, business or another service.
- A short chronology with dates, channels used and exact results.
- The carrier’s case/reference numbers and written responses.
- Clear screenshots of the error, with OTPs, passwords, full ID numbers and unrelated personal data redacted where possible.
- A precise remedy: complete registration, correct the record, explain a refusal, restore the supported process or investigate the service issue.
- Identity or ownership evidence only if the official form or regulator specifically requests it.
How to file the NTC telco complaint
Open the official NTC form
Go directly to ntc.gov.ph/telco-complaint. Confirm the domain before entering account information.
Identify yourself and the provider
The current form asks for the full/account name, network provider, account number, complete/billing address, updated contact number and updated email.
Handle a missing prepaid account field honestly
If you do not have a conventional account number, use only a mobile/reference value the form instructions accept. If the required field remains unclear, ask NTC or a regional consumer office rather than inventing a number.
Write a factual summary
State what happened, when it happened, which official channel was used, what the carrier said and the specific resolution requested. Avoid insults, guesses about motives and unsupported accusations.
Provide only requested evidence
Follow the current official form. Never include a live OTP, password, wallet PIN or unnecessary full identity document in a free-text field.
Review and submit
Check the email and alternate contact carefully so NTC can reach you. Save the acknowledgment, submission screen or reference supplied by the system.
Respond through verified channels
If NTC or the carrier requests clarification, verify the sender and reply with the complaint reference. Keep every follow-up in the same evidence file.

For a scam or spam text
NTC provides a separate Text Scam/Spam Report. Its current form asks for reporter/contact details, the sender’s number, a summary, qualifying identity evidence and an image of the message showing the number.
Do not select the link or reply
Preserve the message, sender number, date/time and visible content.
Capture the evidence
Take a readable screenshot before blocking or deleting. Hide unrelated conversations and one-time codes.
Report through official routes
Use the carrier’s fraud channel and the NTC form. Contact a bank, wallet or law-enforcement authority separately when money, account access, threats or criminal conduct are involved.
For misuse of your registration data
A privacy complaint belongs with NPC, not only NTC. NPC’s rules generally require the complainant first to inform the respondent in writing and allow it to address the alleged violation. The current mechanics state that exhaustion is shown when the respondent fails to take timely or appropriate action, or does not respond within 15 calendar days of receiving that written notice.
NPC currently requires a filled-out and notarized complaint-assisted form or verified complaint with evidence, subject to its current filing rules. Review NPC’s complaint mechanics immediately before filing; do not reuse an obsolete template.
After submitting a complaint
- Keep the acknowledgment and quote the reference in later communications.
- Continue protecting the number and linked accounts while the complaint is reviewed.
- Tell NTC if the carrier resolves the issue; provide the outcome accurately.
- Do not create duplicate complaints through many addresses unless the agency instructs you to do so.
- Do not publish staff names, full account records or identity documents as a pressure tactic.
- Obtain qualified legal advice when the dispute involves substantial loss, alleged crime or urgent legal relief.
